Team-Based Care is a podcast for the interprofessional healthcare team. Produced by the UW-Madison School of Medicine and Public Health - Office of Continuing Professional Development, speakers from different professions explore a topic or theme that is evidence based, relevant to their practice, and has an interprofessional lens.

In this episode of Team-Based Care, we will describe health care transition, discuss ways in which health care professionals can help through the transition period and discuss areas of improvement.  Our guest host for this episode is Tim Markle, Director WI Youth Health Transition (YHTI) and Outreach Program Manager, Children's Resource Center-South.
