Title: Community Health Needs Assessment: Data to Action Addressing Inequities

As a result of participating in this educational activity, learners as members of the healthcare team will be able to:

  1. Describe comprehensive process for leveraging a Community Health Needs Assessment to implement community health improvement activities
  2. Identify multi-sector partners to address social determinants of health through community-clinic collaborations
  3. Describe the design of a care coordination system in partnership with community members and agencies to screen, refer, navigate and connect patients to address social needs and improve maternal and child health outcomes
