Youth to Adult Health Care Transition: Understanding the Basics, Implementation Tools, and the Role of Supported Decision-Making - Julie Hajewski, NP
Certified Nurse Midwives (CNM), Certified Registered Nurse Anesthetists (CRNA), Nurse Practitioners (NP), and Physician Assistants (PA). Additional professionals who would benefit from attending include MD/DOs, RNs, and students.
As a result of this educational activity, learners will be able to:
- Define Health Care Transition (HCT) and Supported Decision-Making.
- Describe the current status of HCT efforts.
- Identify one HCT strategy and one resource to support implementation of HCT in your clinical practice.
- Recognize the role of the health professional, and discuss appropriate situations, in which health professionals can facilitate family decisions regarding tools that promote individual autonomy and support individuals with special healthcare needs.
- Access resources that will support the use of Supported Decision Making tools in practice.
