Objectives:

As a result of this educational regularly scheduled series, learners will be able to:

1. Describe the role of the APP in the LTC setting as vital members of the patient's care team.

2. Explain the CMS regulations regarding PCP and APP visits while a patient is residing in a LTC facility.

3. Discuss preferred providers and ACO patients as it pertains to three-day waiver.
