Objectives:

As a result of this educational regularly scheduled series, learners as members of the healthcare team will be able to:

  1. Define Health Care Transition (HCT) and Supported Decision-Making
  2. Describe the current status of HCT efforts
  3. Identify one HCT strategy and one resource to support implementation of HCT in your clinical practice
  4. Recognize the role of the health professional, and discuss appropriate situations, in which health professionals can facilitate family decisions regarding tools that promote individual autonomy and support individuals with special healthcare needs
  5. Access resources that will support the use of Supported Decision Making tools in practice
