PANDAS and PANS: The Controversy
Pediatric Autoimmune Neuropsychiatric Disorder Associated with Streptococcus (PANDAS) and Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS) are clinical syndromes occurring in children which are characterized by the abrupt onset of neurobehavioral symptoms (most often obsessive-compulsive disorder). PANS is the umbrella term for the disorder. When the instigating event is group A streptococcus, the syndrome is referred to as PANDAS. The biologic plausibility for these disorders is provided by the example of acute rheumatic fever which is precipitated by a throat infection with Group A streptococcus. This activity discusses acute rheumatic fever and Sydenham chorea as a model for PANDAS and PANS and reviews the diagnostic criteria and approach to management of these entities.

Pediatric providers, including physicians, nurses, physician assistants, and other providers interested in learning more about PANDAS and PANS clinical syndromes.
Learning Objectives
As a result of participating in this activity, learners, as members of the healthcare team, will be able to:
- Discuss the clinical spectrum and criteria for the diagnosis of neurobehavioral syndromes, PANDAS and PANS
- Review streptococcal pharyngitis and acute rheumatic fever with concentration on the major manifestation of chorea, for plausibility of PANDAS
- Review the original description of PANDAS and a typical case, and highlight controversial issues that have resulted in skepticism
- Discuss the newer concept of PANS and approach to diagnosis and management
