Series Overview

This conference is meant to provide evidence-based material and presentations to broaden the participants knowledge and management of advanced heart disease. The atmosphere is meant to engage the learner in discussion and self-reflection in order to apply these newly acquired skills/information to their practice in patient care. Topics are determined collaboratively between participating facilities and are presented by the Advanced Heart Disease and Transplant team from UW Health.

The University of Wisconsin–Madison ICEP designates this live activity for a maximum of 1 ANCC contact hour. 

Accreditation Council for Pharmacy Education (ACPE)

A maximum of 1 hour of knowledge-based CE credit can be earned by successfully completing this live activity. Pharmacists should claim only the credit commensurate with the extent of their participation in the activity. CE credit information, based on verification of live attendance, will be provided to NABP within 60 days after the activity completion.

ASWB Approved Continuing Education (ACE)

As a Jointly Accredited Organization, the University of Wisconsin–Madison ICEP is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program.  Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit. Social workers completing this course receive 1.0 General continuing education credits.

The University of Wisconsin–Madison ICEP, as a member of the University Professional & Continuing Education Association (UPCEA), authorizes this program for 0.1 continuing education units (CEUs) or 1 hour.

As a result of participation in this educational series, members of the healthcare team will:

  1. Recognize clinical criteria that requires escalation to a higher level of care in regards to heart failure management
  2. Demonstrate appropriate titration of guideline-directed medical therapy (GDMT) for advanced heart failure patients
  3. Utilize best practices and evidence-based recommendations in heart failure management and consideration of advanced therapies, while also considering any health disparities that may be present      
  4. Prioritize clear and effective interprofessional communication between care facilities in order to address the overall needs of the patient as a team
