Goals of Care in Primary Care
In this online learning module, participants will learn about why it is important to have and document Goals of Care discussions in a systematic way in the primary care setting for patients with complex care management needs.
They will learn about a team-based ThedaCare pilot initiative designed to identify patients at high-risk of misalignments in plans of care and facilitate Goals of Care discussions and appropriate continuing services and/or onward referrals.
Participants will learn how such discussions can be documented and reviewed using a novel Goals of Care flowsheet in the electronic health record, as well as how to have appropriate, comprehensive, compassionate, and patient and family centered Goals of Care discussions.

Learning Objectives
As a result of participation in this educational series, members of the healthcare team will be able to:
1. List at least two reasons why it is important to have and document Goals of Care discussions with patients and, where appropriate, families and/or surrogate decision makers.
2. Describe two circumstances in which having a Goals of Care discussion might be helpful and appropriate.
3. Describe the tool used to document Goals of Care in the electronic health record in a consistent way that is visible to all health providers across settings.
4. Demonstrate improved comfort having and documenting Goals of Care discussions.
