Series Overview
Pediatric Grand Rounds is a weekly multidisciplinary and interprofessional conference designed to improve a participant's ability to practice pediatric medicine, including patient care, conducting healthcare research, providing medical education, addressing public health, and implementing innovation.
Intended Audience
This activity was designed to meet the needs of health care providers who participate in the care of the pediatric and adolescent patients. This includes, but not limited to: MD/DO’s, RN’s, APRN, Residents, Fellows and other Trainees.
Learning Objectives
As a result of participation in this educational activity, members of the healthcare team will increase their ability to:
- Define healthcare transition and explain its importance in facilitating successful transfer of care from pediatric to adult healthcare systems
- Identify barriers to the implementation of effective healthcare transition processes
- Discuss the challenges and benefits of incorporating school nurses into healthcare transition efforts
