Elder Veteran Program (EVP): Creating a Geriatric Sensitive Culture - Maureen Haske-Palomino, DNP, GNP-BC Liza Gatley, DNP, AGACNP-BC Rochelle Carlson, MS, RN, GNP-BC, CRRN
The EVP Care team is a nurse practitioner led inpatient geriatric consult service developed to improve the care of older hospitalized veterans by increasing access to geriatric focused care and empowering nurses to identify risk early and employ patient-centered interventions that focus on preventing hospital acquired functional decline and associated complications. A team of nurses and nurse practitioners meet with at risk inpatient older adults and perform geriatric assessments that focus on optimizing veteran-centered nursing interventions. Additionally the nurse practitioners will see inpatients with active geriatric issues and work with the inpatient medical/surgical teams to optimize inpatient care of older at risk veterans and assist with disposition when warranted.
Learning Objectives
Upon completion of this educational activity, participants will be able to:
- List two components of the Elder Veteran Program that enhance adoption of geriatric nursing practices.
- Identify the importance of proactive risk assessment in hospitalized older adults.
- Describe one barrier & one facilitator of implementing a nurse practitioner led inpatient geriatric consult service.
