Overview

Health care transition (HCT) is the process of preparing youth and young adults for the transition to a healthy adulthood. For many, this means moving from a pediatric to an adult model of health care. This series is for health care providers who work with youth and young adults with intellectual and developmental disabilities. This includes primary and specialty care providers, advanced practice providers (NP/PA), nurses, speech-language pathologists, and any other provider interested in transition. Participants will learn and share strategies to support youth and young adults with intellectual and developmental disabilities to direct their own transition from pediatric to adult care and a healthy adulthood.

Intended Audience

This activity is intended for physicians, nurses, physician assistants, psychologists, social workers, and speech-language pathologists.

Global Objectives

As a result of participation in this educational activity, members of the healthcare team will be able to:

  • Improve their ability to provide best practice care to youth/young adults with intellectual and developmental disabilities with specific conditions.
  • Improve their ability to address the unique needs of youth/young adults with intellectual and developmental disabilities
  • Improve their ability to support youth/young adults with intellectual and developmental disabilities to direct their own transition from pediatric to adult care and a healthy adulthood

Elements of Competence

This continuing education (CE) activity has been designed to change learner competence and focuses on the American Board of Medical Specialties areas of patient care and procedural skills, and interpersonal and communication skills, in addition to the interprofessional competencies of roles/responsibilities, interprofessional communication, and teams and teamwork.

American Nurses Credentialing Center (ANCC)

The University of Wisconsin–Madison ICEP designates this live activity for a maximum of 1.5 ANCC contact hours.

The University of Wisconsin–Madison ICEP has been authorized by the American Academy of PAs (AAPA) to award AAPA Category 1 CME credit for activities planned in accordance with AAPA CME Criteria. Continuing Education (CE) credits for psychologists are provided through the co-sponsorship of the American Psychological Association (APA) Office of Continuing Education in Psychology (CEP). The APA CEP Office maintains responsibility for the content of the programs.
As a Jointly Accredited Organization, the University of Wisconsin–Madison ICEP is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program.
