Overview
Health care transition (HCT) is the process of preparing youth and young adults for the transition to a healthy adulthood. For many, this means moving from a pediatric to an adult model of health care. This series is for health care providers who work with youth and young adults with intellectual and developmental disabilities. This includes primary and specialty care providers, advanced practice providers (NP/PA), nurses, speech-language pathologists, and any other provider interested in transition. Participants will learn and share strategies to support youth and young adults with intellectual and developmental disabilities to direct their own transition from pediatric to adult care and a healthy adulthood.
Intended Audience
This activity is intended for physicians, nurses, physician assistants, psychologists, social workers, and speech-language pathologists.
Global Objectives
As a result of participation in this educational activity, members of the healthcare team will be able to:
- Improve their ability to provide best practice care to youth/young adults with intellectual and developmental disabilities with specific conditions.
- Improve their ability to address the unique needs of youth/young adults with intellectual and developmental disabilities
- Improve their ability to support youth/young adults with intellectual and developmental disabilities to direct their own transition from pediatric to adult care and a healthy adulthood
Elements of Competence
This continuing education (CE) activity has been designed to change learner competence and focuses on the American Board of Medical Specialties areas of patient care and procedural skills, and interpersonal and communication skills, in addition to the interprofessional competencies of roles/responsibilities, interprofessional communication, and teams and teamwork.

Activity Objectives:
As a result of participation in this educational activity, members of the healthcare team will be able to:
- Explain the importance of transition readiness assessment in healthcare transition (HCT)
- Explore a strategy to support sustainability of implementation
- Explore how transition readiness assessment can be used to support HCT planning
