Overview

Health care transition (HCT) is the process of preparing youth and young adults for the transition to a healthy adulthood. This series is for health care providers who work with youth and young adults with intellectual and developmental disabilities. This includes primary and specialty care providers, advanced practice providers (NP/PA), nurses, speech-language pathologists, and any other provider interested in transition.
