  4. Left of page, under COURSE PROGRESS, SELECT Evaluation (Note: Quiz is only required if you'd like to receive IJCAHPO credit.)
  5. PRESS Start
  6. FILL OUT the form then PRESS Submit to finish
  7. Congratulations - you have recorded your attendance and earned your credit!
