Pharmacy Innovations Conference on Antithrombotic Therapy (PICAT) 2026

October 2, 2026

Overview

The yearly Pharmacy Innovations Conference on Antithrombotic Therapy (PICAT) is developed to provide cutting edge science to the target audience of pharmacists whose clinical practice includes antithrombotic patient care. Attendance at this program is targeted toward pharmacists practicing in anticoagulation clinics or teaching in schools and colleges of pharmacy.

Learning Objectives

  • Describe key anticoagulation considerations for patients with impaired gastrointestinal (GI) function, including how GI surgeries or interventions may alter medication absorption and pharmacokinetics, and how these changes should inform anticoagulant selection and clinical monitoring strategies.
  • Describe the importance of a systematic approach to assessing renal function in patients treated with a direct oral anticoagulant, as well as the benefits and trade-offs in using the Cockroft-Gault and CKDEpi formulae for these estimates in clinical practice.

  • Discuss various case scenarios using platelet function testing to guide patient-specific pharmacotherapy decisions

  • Explain how the laboratory and bedside clinicians should actively engage in developing policies, management approaches and related support to manage the use of antithrombotic therapies.

  • Apply the 2026 AHA/ACC PE Clinical Categories (Understand the 2026 AHA/ACC Acute PE Guidelines)
  • Describe some of the considerations and management strategies for anticoagulation that exist in frail elderly adults.
  • Identify the safest common direct oral anticoagulant (DOAC) for treating acute venous thromboembolism (VTE)

  • Evaluate opportunities and barriers for implementing or expanding eGFR-based dosing workflows within their own health-system practice environment.

  • Review the benefits and limitations of using the anti-Xa assay to measure selected anticoagulants.

  • Explain how the laboratory and bedside clinicians should actively engage in developing policies, management approaches and related support to manage the use of antithrombotic therapies.

  • Evaluate how impaired gastrointestinal function affects the absorption, efficacy, and safety of anticoagulant therapies, and apply this knowledge to select and adjust anticoagulation strategies in this patient population.
