Overview 

Providers spend their days diagnosing, treating, and heroically surviving back-to-back appointments while wondering if lunch is a myth. Coders, meanwhile, translate those clinical stories into the highly regulated language of ICD-10-CM, CMS, auditors, and compliance. So when a coder asks, "Can you clarify laterality, acuity, and whether this condition is acute on chronic?" the provider often hears, "I'd like you to reopen this chart and contemplate your life choices." The reality is that neither side is wrong—providers speak medicine, coders speak compliance. That's where Outpatient Clinical Documentation Integrity (OP CDI) comes in. Our role isn't to police documentation or create more work. Our role is to serve as the interpreter—helping to make sure the patient's care and story is communicated clearly,
accurately, and completely so everyone leaves the conversation understanding the same patient. Each quarter, we'll tackle real-world documentation challenges through practical, case-based discussions that translate documentation and coding concepts into strategies you can use in clinic the very next day. We'll demystify the resources designed to support you, including informational HCC Our Practice Advisory (OPAs), post-visit clarification queries, documentation tip sheets, and the collaborative roles of the OP CDI and HCC Coding teams. Think of us less as the documentation police and more as the GPS that occasionally says, "Recalculating..."before anyone accidentally drives the chart into a coding ditch.
