A prolonged extrication that demands coordination across multiple agencies.

The cold adds urgency to an already difficult response. Patient access is limited, the rescue takes time, and clinical needs continue to change.

Fire and rescue personnel, ground EMS, and Med Flight must reassess the plan together while preparing for the transition to air transport.

 

Join us for another session of our subseries: Med Flight Chronicles: “Against the Elements,” a moderated discussion with members of the response team.

We are pleased to welcome Ryan Wubben, MD, to the Emergency Education Night series. A UW Health emergency medicine and Med Flight physician,

Dr. Wubben was part of the responding crew.

We also welcome back Ross Bausman, the Med Flight flight nurse who responded alongside him.

Joining them are Brandon Masbruch, Belmont EMS Assistant Chief and an on scene responder;

 

Cameron Conrad, UW Health Med Flight helicopter pilot

and Kari Sowieja, Med Flight flight nurse and supervisor.

Thomas Ellison, UW Health Adult Trauma Program Manager, will present  a brief case synopsis of the event.

He will also bring the trauma program’s perspective to the discussion, including receiving team preparation and how performance improvement can strengthen care across agencies.

Together, the panel will explore how teams managed limited access, changing patient needs, environmental challenges, and a rescue plan that had to evolve. From the initial response through hospital arrival, they will examine the communication, clinical care, and cooperation that carried this response forward.

Learning Objectives

At the conclusion of this activity, participants will be able to:

  1. Identify how evolving scene information, environmental conditions, and structural damage influence scene assessment, patient access, resource needs, and extrication planning during prolonged entrapment.
  2. Analyze how changes in patient condition, scene constraints, and extrication progress inform the reassessment and adaptation of the rescue plan during prolonged entrapment.
  3. Describe the resuscitation priorities during prolonged entrapment, immediately after release, and during air transport, including hemorrhage control, analgesia and sedation, vascular access, blood product administration, airway management, and hypothermia prevention.
  4. Evaluate how communication and role coordination among dispatch, fire and rescue personnel, ground EMS, air medical clinicians, flight operations, and the receiving trauma team support continuity of care, trauma activation, and system performance improvement.

The overall goal of the Emergency Education Night Webinar Series is to strengthen clinical judgment, communication, and system coordination among emergency and prehospital providers through evidence informed education that supports high risk and time sensitive patient encounters.
