Immunizations for Adults: An Implementation Toolkit for Performance Improvement Projects
This toolkit will guide you through a quality improvement (QI) project that can be used for a Maintenance of Certification (MOC) Part IV process and documentation. The content provides suggestions and resources to help yourself, your clinic, or clinic team to improve the rates of recommended immunizations or achieve another goal related to immunizations in your practice. You are encouraged to tailor the project to fit your needs and use familiar QI tools and strategies.
MOC allows physicians to maintain their board certification through one of the 24 approved medical specialty boards of the American Board of Medical Specialties (ABMS). MOC Part IV focuses on data-based QI in medical practice. Activities to assess and improve patient care and outcomes can be used to maintain board certification through this process.
Content for this toolkit was medically reviewed by Paul Hunter, MD. Dr. Hunter was the Principal Investigator for Pneumococcal Conjugate Vaccinations (PCV) in Adults in Primary Care: A Quality Improvement Project.
The University of Wisconsin–Madison Interprofessional Continuing Education Partnership (ICEP) gratefully acknowledges the quality improvement grant provided by Pfizer.
