Overview

Health care transition (HCT) is the process of preparing youth and young adults for the transition to a healthy adulthood. For many, this means moving from a pediatric to an adult model of health care. This free, virtual, and national series is for health care providers who work with youth and young adults with intellectual and developmental disabilities. This includes primary and specialty care providers, advanced practice providers (NP/PA), nurses, speech-language pathologists and any other provider interested in transition. Participants will learn and share strategies to support youth and young adults with intellectual and developmental disabilities to direct their own transition from pediatric to adult care and a healthy adulthood.

Intended Audience

The target audience for this activity are physicians, physician assistants, nurses, advance practice nurses, psychologists, and social workers.

Global Objectives

As a result of participation in this educational activity, members of the healthcare team will be able to:

  1. Describe the healthcare transition process, including ideal timeline
  2. Access healthcare transition resources for professionals, youth & young adults (Y/YA), and their families

January 14 Session Learning Objectives

As a result of participation in this educational activity, members of the healthcare team will be able to:

  1. Describe the role of school nurses to support health care transition planning for students with disabilities or special health care needs.
  2. Identify  strategies healthcare professionals can use to  support schools and school nurses in student transition to adulthood.

This educational activity is designed to change learner competence and focuses on the following competencies:

  • Accreditation Council for Graduate Medical Education (ACGME)/Nursing: patient care and procedural skills, interpersonal and communication skills
  • National Academy of Medicine (NAM)/Nursing: provide patient/person-centered care
  • Interprofessional Education Collaborative Competencies: roles and responsibilities, communication, and teams and teamwork
  • UW ICEP Diversity, Equity, and Inclusion: engage in self-reflection, address health disparities, and value diversity in the clinical encounter

As a Jointly Accredited Organization, the University of Wisconsin–Madison ICEP is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. Regulatory boards are the final authority on courses accepted for continuing education credit.
