Consider the following before answering the question below and use the template identified above that corresponds to your response to this question:

Note: The format of the journal club/case conference should be discussed to determine the appropriate evaluation strategy. If participants are reviewing the quality of the presentation, use a Grand Rounds format template.

1. Customized: You will have to populate the evaluation for each session where you plan to use it.
   • It WILL be possible to include specific learning objectives and presenter names, if needed.
   • It WILL be possible to make completion of evaluation required for claiming credit.
2. Same: You will create the evaluation survey once, for the entire series.
   • Do you want the responses to be anonymous, which allows the learner to complete the evaluation without logging in?
   • OR
   • Do you want the responses to be tied to the learner, which means the learner must log in to their account to complete the evaluation?
     • It will not be possible to include specific learning objectives and presenter names.
     • It will not be possible to make completion of evaluation required for claiming credit.
